Who genuinely needs Microsoft 365 Business Premium?

Honest answer: any business that would be embarrassed, fined or sued if a laptop were stolen or a mailbox were compromised. In practice that includes:

  • Medical and dental practices that need HIPAA-grade controls on devices and email, and documentation to prove it
  • Law and accounting firms holding client financial data, where a mailbox takeover leads directly to wire fraud
  • Anyone applying for cyber insurance, since insurers now expect MFA enforcement, EDR on every endpoint and the ability to wipe lost devices, which is exactly what Premium supplies
  • Businesses with remote or hybrid staff using personal devices, where Intune is the only practical way to keep company data separated and protected
  • Defense subcontractors and manufacturers working toward CMMC, where Premium is a starting point rather than the finish line

Who can skip it? A three-person shop with no sensitive data, no remote work and no insurance requirement can run on Standard with a separate EDR product. But once you add the third-party EDR, MFA management and email security you would otherwise need, Premium is frequently the cheaper path. That is the calculation we run before every Microsoft 365 migration.

Can you mix Microsoft 365 licenses across staff?

Yes, and you should. Microsoft allows any combination of Basic, Standard and Premium in the same tenant. A pattern that works well for a 30-person Vero Beach practice looks like this:

  • Premium for the owner, office manager, billing, anyone handling patient or financial data, and anyone who works remotely
  • Standard for office staff who need desktop apps but sit at a managed workstation all day
  • Basic for part-time, field or clinical staff who only need email and Teams on a phone
  • Exchange Online Plan 1 or shared mailboxes for service addresses like info@ and billing@, which need no full license at all

One caution: tenant-wide security features such as conditional access apply based on the licensed user. If you protect only the Premium users, the Basic user with a weak password becomes the door in. We usually recommend Premium for everyone with a company laptop and Basic only for phone-first roles that Intune still covers through mobile app policies.

What do the plans not include?

  • Backup. None of the three plans back up your data in the way a business expects. Deleted items age out of retention, and ransomware can sync through OneDrive. Third-party Microsoft 365 backup is a separate, inexpensive line item.
  • Phone service. Teams Phone is an add-on if you want Teams to replace your desk phones.
  • Someone to configure it. Premium ships with its features turned off. Conditional access, Intune compliance policies and Defender settings must be designed and deployed. An unconfigured Premium tenant is a Standard tenant that costs more.

Is Business Premium worth the extra cost over Standard?

If you compare Premium to Standard alone, the difference looks like a security tax. Compare Premium to Standard plus a third-party EDR product, an MFA solution, an email security gateway and a mobile device management tool, and Premium usually wins on price while being simpler to manage.
